The following forms contain information that should be reviewed by both parents/guardians and participants prior to participation. After reading the Participation Policy and Drug Testing Policy (HS Only), both documents contain a form that must be completed and submitted to coach, program director or the Activities Office. All athletes, cheerleaders, dancers, members of Marching Band and Show Choir are required to submit a COMPLETED Physical Form prior to participation. Additionally as part of the Physical Form parents/guardians and participants are required to sign that they have reviewed concussion related materials. There are two links below that provide the required information regarding concussions that should be reviewed prior to signing. Finally, we have provided a copy of the Lawrence E. Smith Supplemental Insurance Policy that all participants are required to have to participate in sports, Cheerleading, Dance, Marching Band and Show Choir.
- per MSHSAA By-Laws the doctor's signature must be dated after 02/01/2017 to be valid for the 2017-2018 school year.
